This article presents information on a report that elaborates on the causes of an accident that took the lives of three people while they were working on the "Apollo" spaceship. The report informed in detail the causes of the mishap, which in this case was thought to be an electrical malfunction. However, the report didn't elaborate on the reasons which gave rise to a situation wherein doubts have been cast over the competency of a reputed governmental agency like the U.S. National Aeronautics and Space Administration.
